Day visitor payments

Use the form below to pay for day visitor access to the Tavistock and Portman NHS Foundation Trust Library

Student access costs £5 and requires valid student ID or documentation. Standard day access costs £10. Prices do not include printing and copying costs, which must be paid for separately with a debit or credit card.

This form must only be completed on the library premises at the start of the day of your visit. If you experience any difficulties, contact our reception desk staff for help.
